In order to determine if your pet is suitable for PermaStay please complete the fields below and click the Submit button at the bottom. We will then acknowledge receipt of your questionnaire with a personalized written report which includes a screen shot of your pet edited to show where PermaStay will be placed and why.  This process takes approximately 45 minutes to complete as your pet is personally reviewed.  It is critical that the information provided is precise. Do not include photo(s) when the ear is standing. The photos should show the pets ear in its most morbid state.  We will provide a referral to a Certified PermaStay Facility in your area or region and will be available to answer any questions or concerns you may have. Your pet’s profile, photos and our evaluation will be provided to the Certified PermaStay facility so when calling for a formal evaluation with the referral surgeon they will know the details of your pet.  A $24.95 fee is charged for this service. However, after you PermaStay your pet a $49.95 rebate will be issued to you.
